1. GUIDING PRINCIPLES FOR GRANTS AND DONATIONS
1.1 What We Support
The Lexington Foundation, Inc. is committed to cancer research, patient care and education that benefit the people of Kentucky.
We will consider grant applications that
- Enhance awareness and education regarding cancer diseases, symptoms, conditions and treatment options
- Provide physical, psychological, or emotional support to people with cancer
- Allow easier, less expensive access to cancer treatment and care
- Advance medical science in the areas of cancer research and care
- Improve the quality of life for Kentuckians with cancer
1.2 Who/What We Cannot Support
- Programs unrelated to cancer
- Programs that do not benefit cancer patient care or advance medical science
- Direct scholarships or grants to individuals
- Political, lobbying, fraternal or social organizations
- Programs that do not benefit primarily the residents of Kentucky
2. INSTRUCTIONS FOR COMPLETING GRANT PROCESS
2.1 Where to Obtain Grant Application Form
The Lexington Foundation, Inc. office at 859-388-2620. The application form can be downloaded by going to Section 5 on the website.
2.2 The Application Form
Five copies of the completed Grant Application form with attachments are required for all grant requests to The Lexington Foundation, Inc. Only an authorized representative of the requesting organization should fill out the form and sign the application.
2.3 Strategies for A Successful Grant Request
- Read the application instructions completely before beginning, follow set format provided, ensure application is typed or printed legibly.
- Visit our web site at www.lexingtonfoundation.org to learn more about our company, mission, values and programs.
- Review our "Guiding Principles for Grants" .
- Submit five sets of the application form with all applicable attachments in a timely manner BY MAIL. Do not staple, bind or use notebooks. Use clips only.
- If application is incomplete or illegible, your organization will be notified of the reasons the application was rejected. Only "complete" applications will be reviewed for approval. Submission of a complete application does not guarantee funding.
2.4 When To Submit The Completed Application
The Lexington Foundation, Inc accepts grant applications January 1st through through June 30th; however, applications for the current year are reviewed in the third quarter and funding occurs in the fourth quarter of the year.

- August 1st through September 30th Applications Reviewed & Applicants Notified
- By December 31st Funding to Approved Applicants

2.6 Questions About The Application Process
Refer to The Lexington Foundation, Inc.’s web site at www.lexingtonfoundation.org for answers to frequently asked questions (FAQ). There are FAQ and Grant sections on the web site. You may also call the Foundation office at 859-388-2620 or FAX your questions to 859-388-2622.
3. REQUIREMENTS FOR SUBMITTING APPLICATION FORM AND REQUIRED ATTACHMENTS
3.1 Grant Application Form
A separate Grant Application Form must be completed for each project. The form can be downloaded from The Lexington Foundation, Inc. web site at www.lexingtonfoundation.org (Grants - Section 5.0) or by calling the Foundation office at 859-388-2620. The form must be signed by an authorized representative of the requesting organization.
3.2 Letter Of Request
Each application should be accompanied by a Letter Of Request. The purpose of the Letter Of Request is to provide more in depth information not included on the Grant Application Form. The letter must be on the requesting organization’s letterhead and must be signed by an authorized representative of the organization. It should include the following information:
- Details of the intended use of the grant money.
- How The Lexington Foundation, Inc. will be recognized for supporting the initiative or project
- Timetable for implementation of the initiative/project
- Objectives of the project
- Measurable outcomes or the project and who will measure them
- Learning Objectives and CME projections if it is an educational program
- Names of The Lexington Foundation, Inc. Board Members or Employees who are Officers of Board Members of the requesting organization. Specify Title.
- Any additional background information to help us understand the funding request
3.3 Budget
Submit a detailed budget listing how funds will be used including itemized costs. Include percent of budget used for fundraising expenses and percent of budget used for administrative costs.

4. LETTER OF REQUEST TEMPLATE
The Letter Of Request may range in page length but should include at a minimum the content as shown below. Please review the requirements for A Letter Of Request in Section 3.2.

The name and address of payee is <name, address of payee and contact
name and telephone number>. The Payee’s Tax ID Number is <tax ID#>.
Provide a description of the need, timeline, objectives, and measurable
outcomes.
Requested funds will be used in the following manner: <Itemized description
of how funds will be used>.
Describe the manner in which The Lexington Foundation, Inc.’s support of
this project or program will be acknowledged.

5. GRANT APPLICATION FORM
The grant application form must be completed, signed by an authorized representative of the organization and be submitted with appropriate attachments as listed in Section 3.0.
